Who we are

Connecting residents across Westchester, Orange & Rockland to free food & social support

Healthy Westchester is a contracted community-based organization (CBO) within the New York State Social Care Network (SCN) — a Medicaid program that leverages federal funding to address the social factors that drive up to 80% of health outcomes, including food insecurity, housing instability, and lack of transportation.

We are on the ground in Westchester, Orange, and Rockland Counties — delivering nutrition support and food access directly to eligible Medicaid members. When a member is identified as food insecure, we're the organization that shows up.

How the program works

Here's how eligible Medicaid members get connected to our free services — from screening to support.

Complete the HRSN Screening

All Medicaid members are screened annually using New York State's 12-question Health-Related Social Needs (HRSN) tool. You can self-screen online or be screened by a community partner like us. It takes just a few minutes.

Eligibility assessment

Once a positive screen is received, an eligibility assessment typically happens within 3–5 days. Our team verifies your unmet needs — such as food insecurity, housing instability, or transportation barriers — and connects you to the right services.

Receive free support

We provide nutrition support and food access directly to you — no third-party referrals needed. Healthy Westchester delivers services to eligible Medicaid members at no cost.

Who qualifies

Eligibility requirements

To qualify for free services through the Social Care Network, individuals must:

  • Be enrolled in Medicaid Managed Care (some services also available to Fee-for-Service Medicaid members)
  • Have at least one unmet health-related social need (food, housing, or transportation) identified through the state's 12-question HRSN screening
  • Belong to a priority population (see below)

Priority populations include

  • Individuals with substance use disorders or serious mental illness
  • People with intellectual or developmental disabilities
  • Pregnant or postpartum individuals
  • Children under 18, including those in foster care or kinship care
  • Individuals recently released from incarceration with chronic health conditions
  • Frequent users of emergency or hospital care
  • Health Home enrollees

  • Home Delivered Meals

    Nutritious, ready-to-eat meals delivered directly to your home — ensuring consistent access to healthy food, especially during times of need.

  • Pantry Stocking

    Assistance stocking your pantry with essential grocery items to support daily meals and ease financial stress.

  • Nutrition Support

    Guidance and education to help families make healthier food choices that fit their lifestyles and traditions — practical advice for everyday life.

  • Kitchen & Cooking Supplies

    Provision of essential kitchen items and appliances to help families prepare nutritious meals at home with ease.

Reimbursed under the NYS 1115 Waiver

All screenings, navigation, and eligible services are reimbursable, meaning they're completely free for qualifying Medicaid members.
